The 2025 tournament entry fee is $600.00 CDN funds. The entry deposit is $200.00. Entries will be accepted and team numbers allocated on a first paid first in basis. ENTRY AND ACKNOWLEDGMENT: I hereby submit my Angler Waiver and enclose the balance of my Entry Fee payable to FORT FRANCES CANADIAN BASS CHAMPIONSHIP INC. By signing this form I accept the Rules and Regulations as set forth and as may be amended at the Rules Meeting and I agree to abide by them. Ialso give my permission for the free use of my name and image in broadcast, telecast, internet or written account of this event. I further understand and agree that the Tournament Committee reserves the right to refund my entry fees if it shall choose for any reason not to accept my registration.
WAIVER OF CLAIM, RELEASE AND INDEMNIFICATION AGREEMENT AND PHOTO AND VIDEO RELEASE: In consideration of the acceptance of my entry in the Fort Frances Canadian Bass Championship I, for myself, my heirs, executors, administrators and assigns hereby waive any claims to which I may become entitled for injury or damage and release Fort Frances Canadian Bass Championship Inc., its promoters, employees, sponsors, organizers, hosts, Directors, Officers, and all other organizers and affiliated organizations, agencies, and/or agents and assigns and any other person assisting in this event from any claims for damages or injury suffered by me as a result of my participation in or travelling to or from this event. I hereby state that I am in proper physical condition to participate in this event and am aware that participation could, in some circumstances, result in physical injury. I further agree to indemnify the Fort Frances Canadian Bass Championship Inc. Its promoters, employees, sponsors, organizers, hosts, Directors, Officers and all other organizers and affiliated organizations, agencies and or/agents and assigns, and any other person assisting in this event from claims or demands arising out of or in consequence of the participation or attendance by me is this Event. As a participant in the Fort Frances Canadian Bass Championship and/or KidPro I acknowledge and agree that any photograph or video images of me may be taken during the course of my participation in the tournament by the tournament organizers, official photographers or media. As a voluntary participant in the Fort Frances Canadian Bass Championship, and/or KidPro, I agree that the photograph, film or video tapes taken of me during my participation in the tournament may be used for tournament promotion purposes by tournament organizers and the media without royalties or prior consent.
